How can I get my free iPhone 4 case from Apple?

UPDATE: Apple has posted an iPhone 4 Case Program website with instructions. iPhone 4 owners must install the iPhone 4 Case Program app on their devices to request a free case.

Those who have purchased their iPhone 4 before July 23, 2010 must apply no later that August 22. Everyone else must apply within 30 days of purchase. To get a free case, your iPhone 4 must be purchased by September 30.

Apple will soon be taking applications for free iPhone 4 cases on their website to alleviate antenna reception problems. The problem has affected some users when the iPhone 4 is held a certain way in areas of weak cellular signal.

Every iPhone 4 owner is eligible to get a free case from Apple until September 30. After this date Apple will evaluate the program and decide how to proceed.

Those who have already purchased an Apple iPhone 4 Bumper Case can receive a refund in lieu of a free case. Refunds are not available for third-party cases. Apple can't produce enough bumpers to cover all iPhone 4 units circulating. The company will select third-party cases from suppliers and offer customers a choice of free cases in the application process.

I ordered a new iphone4 on July 22. As of today, Aug 2, it still shows backordered. I am going out of town Aug. 4 until Aug. 25. My question is, is the purchase date July 22 or is it when my credit card is charged? (Because if you purchase before 7/23, you have only until 8/22 to apply for the free case and I won't be in possession of my phone to download the app until after the 25th)
